Care Benefit

· Choose any covered physician, hospital,etc..

· Benefit payment is 85% of the covered charge after the $100 deductible, Medical Management must be contacted when required

· All payments under the plan reduce the life time maximum of

$1,000,000

· Employee may be responsible for charges in excess of reasonable, or services that are not medically necessary


Mental Health and Substance Abuse Benefit

· In and out of network benefits available

· In network copay is $15.00

· Out of network authorized services are paid at 75% of covered charges after a $100 deductible


Managed Pharmacy Services

· Administered by Merck-Medco,

· Maximum 21 day supply for the retail program

· In network copay is $2 for a generic, $6 for a name brand

· Out of network benefit is 75%

· Mail order is available, $5 for a 90 day supply


Life Insurance Benefits

· Active Life Insurance is $10,000

· Retired Life Insurance is $2,000

· Accidental Death and Dismemberment benefit for active employees is $8,000

· Please update your beneficiary cards when you have a life change, such as a divorce

· Coverage begins after one year of employment

Early Retirement Major Medical Plan

· Eligible with 360 months of service at age 61 when certain requirements are met

· $100 deductible, 80% coverage

· Managed Pharmacy Plan applies

· Coverage continues until the employee becomes eligible for Medicare at age 65. The Plan can not cover any person eligible for Medicare
